A 51-year-old lady came with pain in her left hip and pubic region with left lower limb shortening. She had a history of roadside accidents 2 months back for which she was treated conservatively with traction somewhere else. Preoperative x-ray showed vertical shear pelvic fracture injury with superior migration of left hemi pelvis. On traction stress, the view pelvis was coming back. Thus she planned for anterior and posterior pelvic ring reconstruction. Post-operative x-ray showed anterior and posterior ring stabilization done with plates and screws with the restoration of the hip center as compared to another side. Clinically limb length discrepancy was corrected. Now the patient is able to mobilize with a walker and is comfortable. These pelvic ring injuries required meticulous surgical fixation for good outcomes.
