Old age lady came with difficulty in walking with valgus deformed knee with instability. Her xray showed valgus deformed knee with hudge defect in posterolateral tibia. These complex knees require meticulous preoperative planning in terms constrained prosthesis, augments to build the tibial defect. She underwent semiconstrained knee replacement surgery with bone graft augmentation of tibial defect with stemmed components. Knee has become stable and balanced. Now she is walking comfortably with support and has been discharged. We routinely performed such complex knee replacement surgeries at sant parmanand hospital civil lines delhi.
